PAYMENT POLICY

Payments toward deductibles, co-pays and co-insurance are to be paid at the time of the office visit, prior to services rendered.  All patients who are self-pay are required to pay the full amount of the visit at the time of the service.

What billing or insurance information will I receive?

After your insurance company has processed your claim, if there is a balance on your account, we will send you a statement.  The full balance is due by the due date.
